The enrollment process is easy as 1-2-3:

		Read the terms and conditions. Then click the “accept” button, acknowledging that you understand and accept the terms and condition of the program.
		Complete the application form supplied upon acceptance of the terms and conditions.
		Receive approval and association code via email within 5 business days of submitting the application. Once approved, you are eligible to take advantage of all program benefits.

TERMS AND CONDITIONS

Qwest® will offer Visa® gift cards to members of approved affinity association who purchase qualifying Long-distance services, Wireless services, DSL with MSN® Premium, DIRECTV® or satellite services. For purchasing state-to-state Long-distance service the customer will receive a $25 Visa gift card and the association will receive $15; for purchasing Wireless service (excluding Add-a-Line) the customer will receive a $50 Visa gift card and the association will receive $25; for purchasing DSL with MSN Premium the customer will receive a $40 Visa gift card and the association will receive $40; and for purchasing DIRECTV service, the customer will receive a $25 Visa gift card and the association will receive $25.

To receive these benefits, orders must be submitted through your assigned local markets sales representative.

You must be a new subscriber to the service and keep the service for 30 days to be eligible for the Visa gift card offer from Qwest. Only one reward for each customer purchase, and a maximum of two rewards during the lifetime of the program. Please allow 6 to 8 weeks for delivery of gift card based on 30-day subscription of the product. If Qwest deems that there is any fraudulent activity, you will be terminated from the program. Qwest reserves the right to modify or terminate the program at any time. If you have questions, please contact your local markets sales representative.

Visa® Gift Card: Available only to new customers through the Affinity program. Customer may receive no more than $140 for these services: $25 for state-to-state long-distance, $40 for Choice™ DSL with MSN® Premium (provided by ISP solely for ISP product), $25 for Digital TV and/or $50 for wireless service. One card per product and customer or customer account. Must keep required services 30 days to qualify. Card will be issued to billing address within 4-6 weeks. Cards are valid for 60 days after issuance; after expiration unused portion forfeit, unless prohibited by law. Using card constitutes acceptance of its terms and conditions.
