Simply complete the online enrollment form. Qwest will review your application within 5 business days and respond back to you.

Is there a cost to join the Qwest Affinity Partner Program?
No. Enrolling in the program is free.

When will members receive their Visa® gift cards?
In order to receive the Visa gift card, the customer must keep the qualifying product for a minimum of 30 days. The pre-loaded Visa gift card will arrive approximately six to eight weeks from the date the order is placed.

Where can I use my Visa gift card?
You can use your Visa gift card anywhere Visa is accepted.

How long will the Affinity Partner Program last?
Qwest Communications reserves the right to cancel or modify the program at any time.
